Obesity Surgery

Tuesday, April 3rd, 2018

Obesity is a health hazard. Body Mass Index (BMI) or 37.5kg/m2 or more indicates obesity—a condition that is risk factor to various life threatening diseases like:

  • Diabetes
  • High Blood Pressure (hypertension)
  • Heart Disease
  • Obstructive Sleep Apnoea
  • Stroke
  • Heartburn or Acid Reflux Disease (GERD)
  • Cancer (in women: endometrial, gallbladder, cervical and uterine; in men: colorectal and prostate)

Studies show that the risk of death from these conditions drops significantly after weight loss.

In addition, obesity contributes to numerous other medical conditions, including:

  • Osteoarthritis and Gout
  • Gallbladder Disease and Gallstones
  • High-Risk Pregnancy
  • Hypoventilation (Breathing Disorders)
  • Infertility
  • Psychological Disorders/Social Impairments
  • Incontinence
  • Polycystic Ovary Syndrome
  • Low Back Pain
  • Degenerative Disc Disease

Six months after surgery for obesity, patients often find they no longer need medications for many of these conditions. Also, many women struggling with infertility before surgery find that conception is possible after weight loss.

Indications

Bariatric surgery is a major operation and should be considered only after non surgical weight-loss methods have failed. If you’re considering weight-loss surgery, you must make a serious, lifelong commitment to lifestyle changes.

Our program follows the WHO guidelines for Asian patients for selection. These guidelines have also been endorsed by the Ministry of Health, Government of India.  If you have a BMI (Body Mass Index) of 37.5 kg/m2 or more, or a BMI of 32 kg/m2 or more with illness related to excess weight, and have not been able to sustain weight loss through diet and exercise, you probably are a candidate for weight-loss surgery.

If you do not meet these guidelines, or we find that you are not eligible for surgery for health reasons, don’t despair. Through our intensive non-surgical weight management program, we can help you lose weight by helping you change your eating habits, modify your behaviour and increase your physical activity.

Procedure

The procedure is performed laparoscopically or robotically with 5-6 small skin incisions ranging from 5 mm-12 mm with the patient being under general anaesthesia. The procedure usually takes anywhere between 3-5 hours on an average and the patient is hospitalised on average for 3 days and discharged on day 4 if the surgery is uneventful.

Our multidisciplinary team of bariatric experts will see you through the entire process. We will make certain that you are prepared for surgery, both physically and psychologically, and we will provide the most advanced and current procedures that are right for you. After surgery, we will assist you in committing to the lifestyle changes that will assure optimal success.

Benefits

Many obesity related health conditions may be improved or resolved completely following a gastric bypass surgery. These include Type II Diabetes Mellitus, High Blood Pressure/Heart Disease, Osteoarthritis of weight bearing joints, Obstructive Sleep Apnoea, Gastroesophageal Reflux Disease, Infertility in women, Stress Urinary Incontinence, etc. The lifestyle of these morbidly obese patients improves significantly, especially in social/economic opportunities and physical functioning and appearance.

Cosmetic Dentistry

Tuesday, April 3rd, 2018

Cosmetic Dentistry using lasers (Erbium and Diode) is suggested for…

  • Smile Designing: This involves contouring or shaping a patient’s gums to improve the appearance of his/her smile by safely removing excess gum tissue that may give him/her a ‘gummy’ smile, or make the front teeth appear irregular in size or shape.
  • Depigmentation: Lightening of dark gums is also possible with this technology.
  • Laser Bleaching: It is also known as tooth whitening and can be done to brighten dull and dark teeth.
  • Composite or Ceramic Laminates: These veneers or crowns can be fabricated to mask unsightly stained teeth, chipped or crooked teeth, or to close gaps between the front teeth.
  • Tooth Jewelry: This is a non-invasive, quick and reversible procedure where a coloured Zirconium stone is bonded onto the tooth, which can add dazzle and sparkle to one’s smile.

Dimple Creation

Tuesday, April 3rd, 2018

Dimples are usually considered as an attractive feature and associated with beauty. Now, the technological advancements in cosmetology have made it possible for virtually anyone to have a dimply smile.

Candidate / Indication

Dimple creation surgery can be done on one cheek or both the cheeks. Along with your surgeon, you can decide which would be the most optimal place for the dimples on your face. Both men and women can go for this surgery.

Procedure

Dimple creation surgery is a daycare procedure performed at our hospital. Under local anaesthesia, a small depression is created in the cheek muscle. This is done from inside the mouth, thus causing no external scars. You will be awake and comfortable throughout the surgery. It takes about twenty minutes to perform a dimple surgery.

Result

Besides a little swelling and bruising, there are minimal complications associated with this surgery. A surgically created dimple is prominent for the initial few weeks but later behaves like a natural dimple, when it is visible only on smiling. Patient will be on antibiotics for only a couple of days after the surgery. However, he/she should be able to resume routine activities the following day. Dimple creation surgery is safe and effective, and is also potentially reversible.

Ear Reshaping

Tuesday, April 3rd, 2018

Ideally ears are positioned close to the side of the head. Children with fly away ears often called ‘Dumbo ears’ easily begin to see the difference, between their ears and other childrens’ ears, as young as three to four years old. More often than not, hurtful remarks made by peers begin to scar children psychologically – as young as six years old. Adults who have lived with prominent ears may also be sensitive about their appearance and display it through poor self-image. Promin Auris (outstanding ears) is an inherited condition that affects about 5% of children today.

Candidate / Indication

Any child or adult who has protruding ears may be a good candidate for ear correction surgery. It is best if the child has reached six years of age, because by this age, the ear has achieved 90% of its adult size. An adult of any age, who is in good health, and has the desire to correct protruding ears, is usually a good candidate.

Procedure

To reposition the ears, the surgeon will make an incision in the back of the ear to expose the cartilage. The cartilage is sculpted or folded as needed, and the final shape is secured with permanent stitches. In some cases, cartilage may be removed to give the ear a natural-looking fold. The ears will then appear closer to the head and more symmetrical with respect to the rest of the face.

Otoplasty can be performed using local or general anaesthesia. In many cases, children receive a general anaesthetic to sleep through the two to three hour long procedure. Older children and adults commonly receive local anaesthesia in combination with a sedative and remain awake during the procedure.

Result

After surgery is complete, protruding ears are positioned closer to the head, which helps resolve the distracting appearance. During recovery from the surgery, you should limit activities in which the ears may be accidentally bent or moved. Bandages are placed around the head to hold the ears in their new position, and are switched to a light head dressing after a few days.

Most children can return to school after one week, and adults may return to work within five days. Consult your plastic surgeon for details specific to your surgery. Results are immediately evident once the bandages are removed.

Chin Augmentation

Tuesday, April 3rd, 2018

Chin augmentation may improve the balance and symmetry of the face and create a fuller, more youthful appearance. The surgery can also compliment another procedure, such as a face lift or rhinoplasty, creating overall facial balance.

Candidate / Indication

With inadequate chin bone structure microgenia, your other facial features may look larger than normal. If you have microgenia and are mentally ready for surgery, you may be a good candidate for chin augmentation.

When the chin is very small and an implant cannot help, then a different type of chin surgery called sliding genioplasty may be recommended. You might not be a candidate for chin augmentation if you have extreme microgenia, especially if your teeth are misaligned when your jaws are closed.

Procedure

To complete the augmentation using an alloplastic implant, your surgeon will make an incision inside the mouth along the lower lip. A pocket will then be created in front of the jaw bone between the bone and the skin. The implant is then inserted through the incision into the pocket.

As an alternative, especially for more severe cases of microgenia, the tip of the chin bone can be moved forward. In this instance, the incision is made either inside the mouth or below the lower lip. Your surgeon will carefully select the portion of the bone to advance forward, then he will cut the chin bone while taking care to minimise trauma to the soft tissue. The bone piece is secured in its new position with titanium screws or wires, and sutures are placed to close the incision.

Non-surgical chin augmentation may also be performed using injectable fillers, such as Radiesse, Juvederm or Restylane.

Result

Your treating specialist will provide you with specific instructions to be followed after chin surgery. You will feel some discomfort and soreness, which you can easily control with medication. You might have to stick to a liquid diet for a day or two. You can resume light activity the day of surgery.

You may be asked to keep your head elevated during this time and practice good oral hygiene. Also, in this duration, you must refrain from touching your chin. You may be prescribed antibiotics and pain medications for the first week of recovery. You should be able to return to work and your usual activity within 7 to 10 days. You will probably have the outside bandage removed within a week of surgery.

Blepharoplasty

Tuesday, April 3rd, 2018

The eyes are the most expressive elements of the face. Drooping upper lids and puffy bags below your eyes can make you look older and more fatigued than you are. However, with blepharoplasty, aesthetic surgeon can help you look more like your alert, normal self by delicately removing fat, excess skin and muscle from eyelids.

Candidate / Indication

If you are affected by puffy bags below your eyes, drooping upper lids, or stretched eyelid skin, blepharoplasty is likely an option for you.

Individuals considering the procedure should have realistic expectations from surgery and be in good general health.

Any one or combination of the following conditions may indicate that you are a good candidate for eyelid surgery:

  • Excess skin obscuring the natural fold of the upper eyelids
  • Loose skin hanging down from the upper eyelids, perhaps impairing vision
  • A puffy appearance to the upper eyelids, making the eyes look tired
  • Excess skin and fine, crepey wrinkles of the lower eyelids
  • Bags and dark circles under the eyes, often with a depression along the bony border of the lower eyelids
  • Droopiness of the lower eyelids, showing white below the iris (coloured portion of the eye)

Those with hypothyroidism, Graves’ disease or other thyroid issues, high blood pressure, diabetes, cardiovascular disease or glaucoma may need to consult their eye specialist before making any decisions.

Procedure

A blepharoplasty procedure can be done alone or in combination with other facial plastic surgery procedures like the facelift or brow lift.

Eyelid surgery requires very small incisions near the areas where loose skin and fat have collected around the eyes. The incisions are made following the natural creases of the upper eyelid, and the area below the lashes of the lower eyelid. After the skin is separated from the underlying fatty tissue and muscle, the excess fat and skin are removed. The incisions are then closed with fine sutures.

In select cases, a special technique called the transconjunctivital technique may be used. This is often suitable for patients who display a lesser amount of fat around the eyelids. In this approach, an incision is made in the lower lid, where no skin but only a small amount of fat is removed.

Result

Expect some bruising and swelling during first few days that usually subsides in about
10 days to 2 weeks. Your doctor will provide you with specific post-operative instructions. You may be advised to keep your head elevated for several days, and use cold compresses to reduce the swelling and bruising. You may also have to avoid wearing contact lenses for about two weeks as well as postpone your exercise schedule by about three weeks at least. You should be able to appreciate the results of your eyelid surgery after about two weeks.

Nose Reshaping

Tuesday, April 3rd, 2018

Heredity often determines the shape and size of the nose; however, past injury may also negatively impact appearance and ease of breathing.

Candidate / Indication

A rhinoplasty corrects and perfects many aspects of the nasal structure and is an ideal procedure if you desire to:

  • Change the size of the nose in proportion to the face
  • Change either the size of the bridge, or the size and position of the nostrils
  • Remove bumps or depressions in the profile
  • Correct an enlarged, drooping, hooked or upturned nasal tip
  • Correct asymmetry in the nostrils
  • Correct any breathing obstructions, such as a deviated septum

Rhinoplasty can be performed starting at age 15-16, when the nose is fully developed. A healthy person at any age can undergo this procedure.

Procedure

To reshape the nose, our doctor will make an incision at the base of the nose along the narrow tissue separating the nostrils, or inside the nose. The soft tissue covering the nose is raised away from the cartilage and bone in order to alter the structure of the nose. Excess cartilage may be removed or if cartilage is needed to shape the nose it may be grafted from another area of the body. To alter the size of the nostrils, an incision is made along the natural crease of the nostril.

Some patients may be candidates for temporary nasal reshaping by way of a procedure called a non-surgical nose job. During a non-surgical nose job, Radiesse or other injectable fillers are used to superficially even out nasal contours.

Result

Splints or tubes inserted into the nose support it during healing for the first week. Swelling begins to lessen after a few weeks and the new shape of the nose is evident after six to eight weeks. Healing continues over the next six months and is complete by 12 months with occasional swelling during the first year.

Face Lift

Tuesday, April 3rd, 2018

The face and neck are the most important part of an individual’s overall look, but unfortunately they are the first to show signs of aging. However, you can get a fresh and smooth look by tightening the underlying muscles, re-contouring the skin and by removing the excess fat from your face, neck and forehead.

A face lift provides more dramatic results than non-surgical options, giving the face a rejuvenated and more youthful appearance. This procedure is ideal for common signs of aging such as the presence of deep creases around the eyes and mouth, sagging skin around the midface, and decreased muscle tone with the presence of jowls along the jaw-line.

A forehead lift smoothens the skin, removing expression lines above the eyebrows and just above the nose. The eyebrows are often lifted to give the face a more alert, refreshed appearance. For sagging eyelids, a blepharoplasty procedure may be recommended.

Candidates / Indications:

Most people are good candidates for a face lift if they are in good health. An ideal face lift candidate:

  • Has sagging tissues that can be treated with a facelift
  • Is looking for high-quality, long-term results
  • Is in good general health
  • Is near their ideal weight
  • Does not have health conditions that may increase risks
  • Does not smoke
  • Is not taking medications or supplements that may increase risks

In addition to your health, it is crucial that you have facial aging symptoms that a facelift can treat, such as:

  • Excess or sagging skin in the neck
  • Jowling
  • Marionette lines
  • Hollows under the eyes
  • Nasolabial folds

Procedure

In the traditional face lift, an incision is made in front of the ear beginning at the temple and continuing around the ear to the lower scalp or base of the hair line. Fat is removed or sculpted around the contours of the face and the muscles may be lifted. The skin is set over the elevated contours and excess skin is removed. In some circumstances an additional incision is made under the chin.

However, as an alternative to the traditional facelift, in the limited incision method shorter incisions are made at the temples and around the ears. Incisions may also be made in the lower eyelid and upper lip.

A Neck Lift addresses excess skin beneath the neck and hanging at the jowls. An incision is made in front of the ear lobe, ending at the base of the hairline. Excess skin is removed to give the neck a firmer appearance.

For Forehead lift, incisions are made along the hair line and upper eyelid.

Result

Within two to four weeks swelling and bruising will subside. Healing should be complete after one to two months and incision remarks will fade within a year.

Male Breast Reduction Surgery

Wednesday, March 7th, 2018

Male Breast Reduction Surgery
Male breast reduction, also called gynecomastia treatment, can help remove feminine-looking breasts for a more masculine physique. Most men are candidates for this body contouring plastic surgery procedure.

Indication
Men with enlarged male breasts (gynaecomastia) can undergo this procedure. If you are under 18, it is advisable to wait as gynaecomastia often can be a side effect of puberty and will resolve on its own.

Ideal candidate for male breast reduction should be

  • Is in good general health
  • Is at or near ideal weight
  • Understands the risks and benefits of the procedure
  • Has realistic expectations

Procedure
Most male breast reduction is conducted using liposuction.

Results
With exercise and right diet you can get that desired physique.

Accessory Breasts Removal
Accessory breasts are usually glands or tissues that remain after the development of the breast. Although accessory breasts may be asymptomatic, they can cause discomfort during menstruation, anxiety, cosmetic problems, pain or restriction of arm movement. Accessory breasts can be satisfactorily treated with excision, liposuction or both. Breast reduction and removal of accessory breasts can be performed at the same time.

Breast Lift

Wednesday, March 7th, 2018

Breast Lift (Mastopexy)
Through aging the skin loses elasticity and the breasts begin to sag. Mastopexy involves removal of excess skin to raise the breasts for giving a firmer and younger appearance. A breast lift may also be combined with breast implants or breast reduction surgery to restore or reduce breast volume as desired.

Candidate / Indication:
A good breast lift candidate should meet the following basic requirements:

  • Age 18 or older
  • In good physical and emotional health
  • Not pregnant, and not planning to become pregnant
  • Currently at or near the goal weight

Have at least a moderate degree of sagging of the breasts

Plain Breast Lift: A candidate for a breast lift surgery without augmentation or reduction will not want a change in breast size. This candidate will like the size of her breasts, just not their positioning.

Breast Lift with Augmentation: Women who want bigger breasts with better positioning on the chest wall would probably benefit from breast augmentation. This procedure may be accomplished by augmentation only or with augmentation and breast lift combined.

Breast Lift with Reduction: Almost all women who need a breast reduction will also benefit from a breast lift.

Procedure
There are three variations of incisions in the breast used during this procedure as mentioned below:

  • An initial incision around the areola followed by a concentric incision around the initial one
  • An incision around the areola with another incision down the front of the breast beneath the areola to the fold of the breast (inframammary fold)
  • An incision around the areola with another incision down the front of the breast beneath the areola to the fold, and horizontally along the fold of the breast

Following the initial incision excess breast tissue is removed to reshape the breast. If the skin has become less elastic, excess skin is additionally removed and the nipple and areola are positioned higher, thus, tightening the surrounding skin.

Result
Results are immediately noticeable. During post-operative recovery, the swelling decreases and incision lines fade away. A breast lift does not alter the overall size of the breasts. However, for fuller looking breasts, augmentation using implants may be recommended.
